Hospital Homebound Services

Academic instruction and other services provided to eligible students who are confined to home or in a health care facility for periods of time that would prevent normal school attendance based upon certification of need by the licensed physician, psychiatrist, or advanced practice provider (physician's assistant or nurse practitioner) who is treating the students for the presenting diagnosis.

 

Parents of students who may need HHB services, should contact their child's school counselor to obtain the application or may request the application from their student's case manager if he/she is a student with special needs.
